Laryngeal Nerves Monitoring in Thyroid Surgery

چکیده

In recent years, intraoperative neural monitoring (IONM) has gained widespread acceptance as an adjunct to the gold standard of visual nerve identification and it can be used identify both recurrent laryngeal (RLN) external branch superior (EBSLN). At present, three types IONM are performed: RLN intermittent neuromonitoring (i-IONM); continuous (c-IONM); EBSLN. i-IONM remains most employed method for confirming well mapping before is exposed in surgical field. allows prognostication postoperative function modifying plan – i.e., postponing treatment contralateral side (staged thyroidectomy) if during procedure loss signal occurs. On other hand, c-IONM detection severe combined events warning surgeon about impending injury, modification maneuvers with a potential preventing unilateral injury. EBSLN may at risk dissection thyroid vessels. Therefore, stimulation or without improve rate functional preservation voice following thyroidectomy.

A Closer Look at Laryngeal Nerves during Thyroid Surgery: A Descriptive Study of 584 Nerves

Morbidity after thyroidectomy is related to injuries to the parathyroids, recurrent laryngeal (RLN) and external branch of superior laryngeal nerves (EBSLN). Mostly these are due to variations in the surgical anatomy. In this study we analyse the surgical anatomy of the laryngeal nerves in Indian patients undergoing thyroidectomy. Materials and Methods. Retrospective study (February 2008 to Feb...

Electrophysiologic laryngeal nerve monitoring in high-risk thyroid surgery.

We recently began performing intraoperative electrophysiologic monitoring of the recurrent laryngeal nerve and the external branch of the superior laryngeal nerve during high-risk thyroidectomies. Neuromonitoring can detect stimulation of these nerves and thereby prevent a mechanical or thermal injury that can result in neurapraxia or axonotmesis. Monitoring is also useful during dissection in ...

Laryngeal nerve monitoring and minimally invasive thyroid surgery: complementary technologies.

OBJECTIVE To determine the feasibility of the combined use of laryngeal nerve monitoring and minimally invasive thyroid surgery. DESIGN Prospective, nonrandomized analysis of single-surgeon experience. SETTING Academic institution. PATIENTS Consecutive series of patients undergoing both minimally invasive thyroid surgery and laryngeal nerve monitoring. MAIN OUTCOME MEASURES Incision len...

Evaluation of the learning curve for intraoperative neural monitoring of the recurrent laryngeal nerves in thyroid surgery.

UNLABELLED Intraoperative neuromonitoring facilitates identification of the recurrent laryngeal nerves (RLN) and allows for predicting their postoperative function. Nevertheless, the outcome of thyroid surgery monitoring is affected by both the experience of the operator and his mastering of the technique. The aim of the study was the assessment of the learning curve for intraoperative RLN neur...

Intraoperative electrophysiologic monitoring of the recurrent laryngeal nerve during thyroid and parathyroid surgery: Experience with 1,381 nerves at risk.

OBJECTIVES The electrophysiologic responses of 1,381 recurrent laryngeal nerves (RLN) during monitored neck surgery were recorded and reviewed. STUDY DESIGN Retrospective case series. METHODS With institutional review board approval, we reviewed thyroid and other neck surgeries performed with intraoperative nerve monitoring (IONM) between the years 1995 and 2006. This list yielded consecuti...
